Advanced Medical
Directives
The U.S. Congress passed the Patient Self-Determination Act in 1990.
Since that date, the conversations about the existence of an Advanced
Medical Directive occur every time you see a new doctor, enter a
hospital, a home health agency, or a hospice. Every person over the
age of 18 who has feelings about quality vs. quantity of life, would
greatly benefit from completing some type of Advanced Medical Directive.
Types of Advanced Directives
Durable Medical Power of Attorney- In this document
you name an individual (referred to as your agent) who you appoint
to make medical decisions for you if you are not able to make decisions
for yourself. It is very important to talk to your doctor, your agent,
and all important family members about your medical choices.
Living Will- In this document you tell your doctors
what care you do or do not want if you are terminally ill. Two doctors
must certify that you are terminally ill before this document goes
into effect.
CPR Directive- This document allows you to refuse
the administration of Cardio Pulmonary Resuscitation (CPR). This
is one of the few medical treatments that is done without your consent
unless you clearly state before hand in writing that you do not want
it.
Five Wishes- This is the most powerful of the Advanced
Medical Directives. It has five parts, or wishes. The first is an
appointment of your agent. The second is an expanded Living Will
and CPR Directive. The third, fourth, and fifth wishes cover your
personal, spiritual and emotional wishes. This document allows you
state your wishes for the medical care and the environment in which
you receive that care.
